Below you'll find the most common forms in use by the Office of University Financial Aid. Some of the forms are required by the office as part of a student's initial application for Financial Aid, while others are requested by the office on a case-by-case basis as documentation of special circumstances. All information provided to the Office of Financial Aid by the student, or by the student's parents or spouse, is completely confidential.
Although there are a large number of forms listed below, only a small fraction will apply to a given student. Please do not submit all of these forms. As a general rule, if you don't know what the form is, or you're not certain it applies to you, please do not submit it.
The Office of Financial Aid notifies students of the status of their Financial Aid Application via e-mail. Any documentation required by the office will be listed on this message and should be sent to the following address:

Application Checklist
This form contains general instructions regarding application for Financial Aid at Case Western Reserve University and a checklist of application requirements for your convenience. While this form should not be submitted to our office, you may find it useful for keeping track of your application for your own records.
Child Support Verification 2013-14
The Department of Education randomly selects a portion of FAFSA applicants for a process called Verification. If you are notified by our office to submit this form, that means that your record is selected and you must submit the Child Support Verification 2013-14 Form.
CWRU Financial Aid Form
The CWRU Financial Aid Form is required for enrolled students. The form is available online through the My Financial Aid Web application. In order to complete the form, login using your CWRU Network ID.
Financial Re-Evaluation Statement
Typically only submitted when specifically requested by a Financial Aid Counselor, this form is completed by a student (and parents if the student is dependent), to document the type and circumstances of any large change in the student's and/or parents' financial circumstances. In such a situation, the Financial Re-Evaluation Statement should be completed and returned directly to the Office of University Financial Aid.
Free Application for Federal Student Aid (FAFSA)
The FAFSA is the primary financial aid form for CWRU and most other universities across the country. The U.S. Department of Education, the sponsors of the FAFSA Form, requires its use by any educational institution disbursing federal financial aid.
High School Completion Status
The Department of Education randomly selects a portion of FAFSA applicants for a process called Verification. If you are notified by our office to submit this form, that means that your record is selected and you must submit the High School Completion Status 2013-14 Form.
Internal Revenue Service Forms
If a student is selected for verification by the U.S. Department of Education or CWRU, he or she must submit a copy of the complete IRS Tax Transcript for 2012 and W-2 Forms to the Office of University Financial Aid. Married students filing separately should submit copies of the spouse's IRS Tax Return Transcript for 2012. Dependent students should submit copies of the parents' IRS Tax Return Transcript for 2012. Families are encouraged to utilize the Data Retrieval Tool on the FAFSA website when completing the FAFSA and not change any of the transferred figures. This will eliminate the need to obtain copies of the IRS Tax Return Transcripts. If the Data Retrieval Tool cannot be used, the IRS Tax Transcript can be obtained at www.irs.gov. On the home page, select “Tools.” Select “Order a Return or Account Transcript.” Follow the instructions for online ordering. You may also request the Transcript by phone at 1-800-908-9946.
Review the documents that are required on My Financial Aid à Documents. Once you receive your Tax Transcript from the IRS, submit the forms in their entirety. The Tax Transcripts and W-2 Forms require submission of the cover page available below for each document.

When parents file a joint Federal Tax Return, the Parents Tax Return Transcript must be submitted. When parents file separately, both the father’s and mother’s Tax Return Transcripts are required.
Applicants for the 2013-2014 academic year should submit the forms from tax year 2012. In addition to the complete Federal Tax Return Transcript, W-2 forms must be submitted.
Students who have not and will not file a Federal Income Tax Return should submit a Student/Spouse Affidavit of 2012 Income. Parents who have not and will not file a Federal Income Tax Return should submit a 2012 Parent Non-Filer Verification Statement.
Memorandum of Assistance (MOA)
The Memo of Assistance Form is required of all graduate/professional student aid applicants. Unlike most other forms, it is not completed by the student. Instead, it is completed by the graduate department for the program in which the student is enrolled. A graduate department representative completes the form, certifying the following information:
- Details of the degree and program,
- Exact number of credit hours and tuition fees charged for the academic year, and
- Type and amount (if any) of graduate financial assistance being received by the student through the department in question, and forwards it directly to the Office of University Financial Aid.
This form is required of all graduate/professional students applying for aid at CWRU, even if said student is not receiving any financial assistance from his or her department. If you are not certain that the form is going to be sent by the University Department in question to the Office of University Financial Aid, please contact the department to arrange this.
Parent Non-Filer Verification Statement
The 2012 Non-Filer Verification Statement should be completed by the parents of a dependent undergraduate, provided the parents have not, and will not, file a 2012 U.S. Income Tax Return (1040/1040A/1040EZ).
Proof of Citizenship
The Department of Education requires our office to verify your Citizenship status. You are required to submit a copy of your US Passport along with this document.
Proof of Permanent Residency
The Department of Education requires our office to verify your Permanent Residency status. Â You are required to submit a copy of your United States Permanent Resident Card (USCIS I-551) along with this document.
Selective Service Registration
The Department of Education requires our office to verify your Selective Service Registration. You are required to submit a copy of your Selective Service Registration Card or registration proof from the Selective Service website www.sss.gov along with this document.
Statement of Educational Purpose/Proof of Identity
The Department of Education randomly selects a portion of FAFSA applicants for a process called Verification. If you are notified by our office to submit this form, that means that your record is selected and you must submit the Statement of Educational Purpose/Proof of Identity Form.
SNAP Verification
The Department of Education randomly selects a portion of FAFSA applicants for a process called Verification. If you are notified by our office to submit this form, that means that your record is selected and you must submit the Supplemental Nutrition Assistance Program Benefit (SNAP) Verification 2013-14 Form.
Student/Parent Untaxed Income Worksheet
The Untaxed Income Worksheet is not required as an initial part of the student's application for Financial Aid. If a student or parent reports untaxed income on the FAFSA Form (Questions 44, 45, 78, or 79), this form will be requested by the Office of Financial Aid. Please note that the student's signature is required even if it was the parent who reported untaxed income.
Student/Spouse Affidavit of Income
This form should be completed by the student (and his or her spouse if applicable) who have not and will not file a 2012 Federal Income Tax Return (1040/1040A/1040EZ). Students will only be asked to submit this form if they were selected for Verification.
Student Petition
The Student Petition Form is a catch-all for any type of special request or supplemental information a student wishes to submit to the Office of Financial Aid. The student should complete the contact information at the top of the form, then describe the nature of, and reasons for, their request in the body of the form. Please note that if your petition results from a change in family circumstances such a death, disability, divorce, separation or unemployment, you should instead submit the Financial Re-Evaluation Statement. The form should be completed and sent to the Office of University Financial Aid.
Study Abroad Cost Sheet - Undergraduate
This form should be completed by students participating in the Study Abroad Program.
Study Abroad Cost Sheet - Graduate
This form should be completed by graduate students studying abroad. Please contact the University Financial Aid Office at financialaid@case.edu if you need a copy of this form.
Verification Form
The Department of Education randomly selects a portion of FAFSA applicants for a process called Verification. If a student is selected, he or she will have this document listed as a required document.
The Verification Form is a five-page document that contains much of the information reported on the FAFSA. Students requiring a Verification Form should review the information (along with parent/s or spouse if appropriate), and then complete and return as indicated on Page 1. Â As part of the verification process, you (along with parent/s) or spouse if appropriate) are required to submit copies of the most recent tax year's IRS tax transcript and W-2 forms as supporting documentation. FAFSA Question 23 Student Aid Eligibility Worksheet
This form should be completed by students who answer FAFSA question 23 as "Yes" or left it blank.
